Are you a new patient? Here’s what you can expect during your first visit with us.

 

We understand you might be apprehensive about your first physical therapy appointment. But from the moment you set foot in our clinic, you’ll feel like you belong at MK Physical Therapy & Wellness. Our welcoming staff is happy to help you begin your path to recovery and answer any scheduling or billing questions along the way.

What should I bring and wear to physical therapy?

 

Please wear comfortable clothing to your appointments, such as athletic wear (shorts and sleeveless tops are preferred). Please do not wear jewelry. For your first appointment, please bring:​

  • Prescription (script)/doctor referral

    • A script may be written by a physician, dentist, chiropractor, podiatrist, physician's assistant or advanced nurse practitioner ​

  • Complete intake form that will be emailed to you once you're scheduled 

    • The email will be sent to you by IntakeQ​

  • List of medical history and medications

What will my first physical therapy appointment include?

 

Your first visit will last approximately two hours and will include a comprehensive evaluation and applicable treatment administered by a licensed physical therapist. Your physical therapist will review your medical history, diagnostic tests and any recent events that may have contributed to your current condition. Your evaluation will include an assessment of your current functional deficits, pain level and posture, as well as a thorough evaluation of your flexibility, strength, balance and endurance.

Following your evaluation, your physical therapist will thoroughly explain your personalized treatment plan and expectations for recovery, 

Do you accept insurance? What is a superbill?

 

MK Physical Therapy & Wellness does not accept insurance directly. However, a superbill can be filed with your insurance carrier for full or partial reimbursement. 

A superbill is used by healthcare providers as a primary source of data for creating claims. These claims will eventually be submitted to patients for reimbursement. Essentially, a superbill is an itemized list of all services provided to a client. The superbill will also contain additional information about the patient visit including practice information, CPT codes, ICD-10 codes, referring doctor and more.

MK Physical Therapy & Wellness is located on the Northern side of the Mission Square Shopping Center, just south of the gas station. Our front door faces north, towards the gas station and Loop 250.
